I assume ultimate responsibility for any loss or damage to any books checked out by the user listed above. I understand that I will be responsible for the cost of replacing any lost or damaged item. I also understand that a $35 processing fee must be paid for any lost or damaged item in addition to the replacement cost. I also assume responsibility for all database activity.
